Patrick A Kazmierczak, 64 of West Salem died Sat January 2, 2016 at Ohio State University Hospital in Columbus.
Memorial Service will be Friday January 15, 2016 @ 7:00 p.m. at the Matteson Funeral Home in West Salem with Pastor Floyd Costello officiating with visitation one hour prior to the services. Online condolences may be shared at the funeral home website @www.mattesonfh.com.

He was born June 11, 1951 in Lodi the son of Chester and Marjorie (McVicker) Kazmierczak.
He graduated in 1969 from Northwestern High School and 1973 from Kent State University.
Pat had been employed at ArtiFlex Manufacturing in Wooster for 34 years. He was a boy scout leader for troop 65.
He is survived by his wife, Jo Anne (Bigley) Kazmierczak whom he married Sept 4, 1981, children, Lisa Ogden of Salt Lake City, Utah, Matthew Kazmierczak of Akron and Jennifer Kazmierczak of the home and his twin brother, Paul (Debbie) Kazmierczak of Medina.
His parents died previously.
